Form to be filled up in capital letters, as accurately as possible.
All information to be duly supported by clear photocopies of documents,
which may be sent by mail upon request.
For uniformity, the photocopies should be made on A4 size paper.
NIC copy of your Next of kin is to be attached.
Two of your recent passport size photographs to be attached.
Mailing address and telephone contact should be accurate, for correspondence from Entice Shipping.
When submitted online or to our office, an application from is thoroughly checked for deficiency or lack of information. A letter of acknowledgement is then
sent to the applicant at his mailing address or by email. You may also find out the status of your application
by email or phone.
